The difference is paper size
A4 and US Letter use different page dimensions. A pattern designed for one format may not print correctly on the other unless the file is specifically supplied in that format.
How to avoid a scale problem
Select the exact paper size in your printer dialog, set the scale to 100% and check the file instructions. Do not force a US Letter file onto A4 paper or rely on a preview thumbnail.
